The State Employee Group Insurance Program (SEGIP) provides Minnesota Advantage Health Plan members (18+) diagnosed with diabetes access to reduced out-of-pocket costs for high-value medical and pharmacy services that are primarily for diabetes. Eligible medical services include physician office visits, dietitian office visits, diabetic retinal eye exams, lab tests, diabetic testing supplies, and pharmacist consults. Eligible pharmacy services include diabetic testing supplies as well as diabetes, hypertension, cholesterol, and depression medications.
Benefits
Waived or reduced out-of-pocket member costs for eligible medical services primarily for diabetes, diabetic testing supplies, drugs, and medication therapy management.
